Supported Housing What is Supported Housing? Torfaen Mind manages 4 shared houses available to adults who are recovering from mental health issues and who need housing related support. The aims of supported housing are to enable residents to be confident in all aspects of maintaining a tenancy and to use community services relevant to their needs. A tenancy with Torfaen Mind is not for life. When residents are confident that they can maintain a tenancy, we support them to find and secure their own long term accommodation. Support staff are available to continue a lower level of support for a period of time once they have moved on. Short term ‘Intense Support’ House Our short-term ‘intense support’ 4 bedroom shared house is situated within easy access of Pontypool. It provides a higher level of staff support than our other 3 houses. The house is staffed for a few hours in the morning and early evening Monday to Friday, and individual support is also provided to residents during the week. Residents accepted into this house should be confident in being ready to move into low support accommodation or their own tenancy within 2 years. Longer Term houses We have three shared houses, all with 3 bedrooms, which provide a lower level of support than our short term shared house. One is located near Pontypool and two others in Cwmbran. Applicants for these houses should be able to demonstrate lower housing related support needs. Staff are not ‘on duty’ in these houses, but provide individual support to residents Monday to Friday during office hours. All residents should be willing to eventually move on into their own accommodation and staff will assist them with finding accommodation and provide ongoing housing related support for a period of time after moving on. Out of Hours Support A 24 hour 7 day a week ‘On-Call’ service is available for all residents. The ‘On-Call’ person will be available to contact any resident who requires emergency support out of hours. Who is eligible for the service?

How to Apply for Accommodation with Torfaen Mind Please contact Torfaen Mind’s Supported Housing team for an application form and risk assessment. You can telephone on: 01495 768830 You can email us at: paulkimbertorfaenmind@hotmail.co.uk In order for the referral to go ahead, we will need a completed application form and risk assessment. We will then contact you to arrange an assessment of the person’s support needs. Ideally, someone who knows the applicant well should be present during this assessment.
